Download the demo and see if that works fine. That way you'll know if it's just your copy of the game or the system that messes something up.

Try updating your sound drivers (and your graphics drivers, while you're at it, though that's not likely to be the problem here). Some early version drivers for Vista were fairly flaky.
This one got solved in another thread. I'll post the solution here as well in case someone else needs it.

I joined the forum so I could share this. I downloaded TLJ off of Steam, and I was having terrible cutscene and dialogue stuttering before making a few changes. Here is some info about my system...
GPU: Nvidia 8600M GT
OS: Windows Vista 64-bit (yeah...I know)
And here are the changes I made...
First, I went to...
Run -> %APPDATA%\The Longest Journey -> Preferences
I turned anti-aliasing off from my video card options.
I then went to...
Control Panel -> Sound -> Speakers -> Speaker Properties -> Enhancements -> unchecked all boxes
and then to...
Control Panel -> Sound -> Speakers -> Speaker Properties -> Advanced -> unchecked all boxes under "Exclusive Mode"
The whole game works perfectly now for me. Hope that helps someone else!
I also went to...
Computer -> C: -> Program Files -> Steam -> steamapps -> common -> The Longest Journey -> configure -> and checked "Force single buffer" and "Force 16-bit graphics" and selected "Windows Multimedia" as the audio device.
